New Toilet on 2nd Floor

My wife and I are buying an old townhouse, and want to install a toilet on the 2nd floor.  The house currently has toilet on the 1st and 3rd floor. 

Have anyone done this before?  How much would it cost approximately? 

And do we have to obtain any approval from URA, or require a civil engineer to sign off etc?
