He hadn't searched for anything like it and was actually confused by what he saw. That made me realize how quickly kids can encounter things accidentally. Similar thing happened at my sister's house, her son was watching normal gaming clips and somehow a genuinely disturbing video showed up in his suggestions. He clicked before realizing what it was and came running to her looking pretty shaken. She ended up setting up Restricted Mode and turning on Google Family Link for content filtering, which caught a lot going forward. We also started using uMobix phone tracker app so she'd get pinged if something questionable slipped through the filters anyway, since algorithms aren't foolproof. Combined that with talking to him about why weird stuff sometimes shows up and that it's not his fault if he stumbles onto something, settings help but kids also need to know it's safe to speak up immediately instead of just closing the tab and staying quiet about it.
